I recently bought a super cheap used offset smoker—you know it’s cheap when the temperature gauge doesn’t have actual numbers on it!

However, it doesn’t have a stem, the whole thing just sort of twisted in, so when I take it out to replace it, the hole it leaves behind is huge (about 2.5” wide). How can I install a regular temp gauge with that hole there? Do I need some sort of patch?

I don't know, but IMO those things are so inaccurate, I would either put it back in to close the hole, or patch the hole.
Then get a Digital Wireless, like a Maverick ET-732, and use that for the Smoker Temp & the Meat Temp.

Recommend having a patch welded in and then drill for a TelTru

I agree with the Tel Tru but not having someone weld a plug.
Put it back in and get yourself 2 Tel Tru therms drill them at grate level, one on each opposite corners of the door, now how have away of controlling the heat.
