When you say you've changed the regulator, was that just the regulator or was it the regulator / hose assembly? Provided that you have a good regulator I doubt that changing the hoses (if you didn't already) would help.
It seems to me the answer is in your air intake someplace. Since the tanks work fine with the gas grill which most likely has a large amount of air intake around the burner(s) I don't think it is the gas (rich or LPG butane rich).
Does the burner have a gap around it to allow air flow around it? Are there any air intake dampers at the bottom of the smoker? I haven't looked closely at the Smoke Vault set up.
One thing I came across is that the new burners sometimes have an oil build up from the manufacturer. Did you check it or clean it before installing it?
There are still plenty of options - if it worked once, it will work again. You might have to make it a better design by increasing any gap around the burner or lower the burner to see if that helps.
