I'm clueless, also, but if I had to take a stab at a guess, it looks like a tool that might be used to twist a pair, or small group, of wires together.
 
Ok now that the cat is out of the bag, the one I made was used primarily to stitch pork loins together to make Crown Roast. He wanted it a bit longer, so I used a 12 inch length of 303 stainless steel. Cross drilled a hole about 1 1/2 inches from a lathe sharpened end and set into a file handle. I guess the needle was about 9 inches not including the handle when done.

 Very nice job on the ebony and antler handle. Alot nicer than my wooden file handle.

The next batch of these will be made from stainless.  I made this one to recreate how we made them when I was a kid.  Except the handle, as a kid the handle was usually part of an old broom or mop handle.

We tended to "lose" them prior to slaughter time thinking we would get out of stringing, but my uncle always made more.  A few years before he died he showed me how to hammer out and shape the tip, hot punch the hole, and then burn in the handle.  The needle was then set into the handle with a JB Weld type epoxy.

For this one I drilled the handle, but hot punched the hole on my anvil with a punch.
 
Dang, I was going to guess prison shank!!    That's a really clever idea though, I haven't seen that before!  Love the handle! 

Well, the secret is out now! Don't guess I'll mail any to the local lockup! :laugh1:

Making handles for these and my knives is my favorite part in the shop! I always know what metal will look like when done, but wood and antler always surprise!
