I print recipes from the Internet and then put them in a 3-ring binder (infinite supply left over from the kid's schooling). I then write notes directly on the recipe, or on the back of the recipe page. For really long notes (like pulled pork, where I log temps every hour) I'll put them on a separate sheet, and stick that in the binder directly after the recipe.

Speaking of school, I was always taught to take notes directly into the textbook. Given that, I don't know why I didn't start writing directly into my cookbooks. I now do that all the time for recipes that come from books. Things like "don't preheat the oven until step 6," or "use 1/4 cup instead of 1/3 cup."
