The "small hole" grinder plate that came with my KitchenAid is 4.5 mm. I used that tonight for the second pass on my fatty. (Pics in the fatty sub-forum.)

I usually don't use a grinder plate with holes that small because I like a more rustic look to my sausages, but it would be useful as a pre-emulsifying grind, I bet!
