I know this is a stupid question and I should know the answer but.........

Do you remove the hard layer (skin?) of fat on the top of the pork shoulder when you make sausage?

Definitely remove the skin!
How much of the fat you remove or retain depends on how fat you want your sausages.

YES...boiled & cleaned then deep fried and seasoned.   Follow enclosed link.
