Last week I bought a New Braunfels BD as a restore project for my son. He’s been sanding off the external rust by hand... we are at the point of diminishing returns.

Can we build a fire in the fire box and burn some of that internal and external rust away? And then use some high temp paint? Or other suggestions?

Personally I would use a wire brush on a grinder or drill get what I can inside and outside, rinse then start a fire to dry the smoker.
Then I would re-season the inside using either Pam spray or vegetable oil applied with a paper towel then start another fire to cook it on and I would keep an eye on that area you might need to oil and heat it more than once.
For the outside you can use hi temp paint or cooking oil or Pam on the outside personally I just use the high temp paint on the outside of my firebox and repaint as needed. If you use oil or Pam you would need to fire it up to "cook it in"

Thanks Piney,

I think I’ve been able to get as much rust off as I can without hardcore grinding... and I guess some rust is inevitable as I live near the coast, and we get morning moisture almost daily.

I have some refined coconut oil that has a very high smoke point. And then run some wood in it. Then paint it. Thoughts?
