There never was a grease fire issue in my opinion. People just didn't understand how to use the grill. It's pretty simple to clean it before/after ever cook or use a drip pan for fatty cuts.

I think hanging right over the coals would provide the best flavor but theoretically as long as the drippings are hitting the lit coals you should get the same flavor profile.
