I've cooked a few pork shoulders & briskets on my PBC. They always turn out nice but yesterday I was a bit flummoxed when it came to hanging my two shoulders.
The fire was pretty hot & after putting the hooks in either side of pork shoulder, the hooks were facing in opposite directions.
The problem this caused is that one hook has the be passed under the bar & same when you remove meat.
It would be so much easier if hooks were opposite (one right & on left) so they be facing the same way when you go to hang meat.

I don't remember having this problem last time. What am I doing wrong?

Did you hang them across the 2 bars or one shoulder on each bar?
